Job summary

One Senior Care seeks a Master Social Worker to guide participants and families through care plans with empathy and expertise. You will engage in assessments, develop care plans, and connect clients with resources, while collaborating with an interdisciplinary team.

We value warm communication, advocacy, and documentation as you support seniors in navigating challenges and maintaining independence, with onboarding and training provided.

Qualifications

• MSW from an accredited program with at least 1 year working with frail/elderly populations.
• Valid state driver’s license and reliable transportation required.
• CPR and First Aid certification or willingness to obtain after hire.

Responsibilities

• Build trusting relationships with participants and families through warm, empathetic communication.
• Participate in initial assessments, care planning, and re-evaluations as part of the interdisciplinary team.
• Provide individual and family counseling with a supportive, person-centered approach.
• Lead group discussions and programs to strengthen emotional well-being.
• Advise participants on community resources and advocate with outside agencies.
• Maintain case management records and complete timely documentation.
• Collaborate across departments to support team cohesion and quality initiatives.
