Fresenius Medical Care seeks a skilled Social Worker to provide psychosocial services to in-center and home dialysis patients. You will work with the healthcare team to promote adjustment, rehabilitation and quality of life, educating staff on emotional and behavioral impacts of CKD.

You will assess patients, participate in care planning, and support transplant referrals while ensuring adherence to FMCNA policies and quality goals.

Competenze

• Masters in Social Work required and state license.
• 2–5 years’ related experience in a healthcare/social work setting.
• Experience with psychosocial assessment and care planning.

Mansioni

• Assess psychosocial status and plan care as part of comprehensive patient assessment.
• Provide monitoring and interventions to support adjustment to dialysis and improve quality of life.
• Educate patients and families about psychosocial issues related to ESRD and available support services.
• Collaborate with the interdisciplinary team on transplant referrals and follow-up.
• Coordinate referrals to community resources to facilitate outcomes.
