Fresenius Medical Care is seeking a dedicated Social Worker to provide psychosocial services for in-center and home dialysis patients, collaborating with the interdisciplinary team to support adjustment and quality of life.

The role conducts assessments, counseling, education, transplant referrals, and connects patients with community resources while adhering to FMCNA policies and quality improvement initiatives.

Qualifications

• Masters in Social Work required.
• Must have state required license.
• 2–5 years related experience in social work or healthcare setting.

Responsibilities

• Assess patients' psychosocial status and plan care.
• Provide counseling to patients and families.
• Educate patients about treatment options and transplant process.
• Identify community resources and coordinate referrals.
• Support Staff with psychosocial education and policy adherence.
