Mary Washington Healthcare is seeking a Case Manager, Discharge (MSW) to organize and expedite a treatment plan for medically complex social issues related to hospital progression of care. The role identifies discharge needs, develops discharge plans, and coordinates with care teams and community resources.

The ideal candidate has a Master’s in Social Work, at least one year of social work experience or a relevant internship in Care Coordination, and proficiency with Windows-based programs.

Qualificações

• Master’s degree in Social Work required.
• Minimum of one year experience in social work or two‑semester internship completed within MWHC Care Coordination.
• Case Management Certification strongly desired.
• Experience with Windows-based programs, e-mail, and Microsoft Word required.

Responsabilidades

• Coordinates care of patients with clinical partners; helps patients advance towards realistic and desirable outcomes.
• Serves as a subject matter expert and resource for care team members.
• Mentors new social work Associates and interns in Care Coordination.
• Assesses long-term and future patient care needs by identifying changes in independence or functional quality.
• Communicates status updates regarding treatment plans with clinical partners.
• Provides information and education on community resources to patients and families.
