Qualifications

Responsibilities

• Coordinates discharge plans with care team and family.
• Acts as mentor for new social work Associates in Care Coordination.
• Assesses long-term care needs and functional independence.
• Communicates treatment plan status with clinical partners.
• Intervenes in cases of child/elder abuse/guardianship as needed.
