Merit Health River Region in Vicksburg, MS is seeking a licensed Master Social Worker (MSW) to provide psychosocial assessments, counseling, care coordination, and discharge planning across the healthcare continuum.

The MSW will collaborate with an interdisciplinary team, support patients and families, and connect them with community and financial resources while maintaining regulatory-compliant documentation and contributing to quality improvement efforts.

Qualifications

• Master's Degree in Social Work (MSW) from CSWE-accredited program required.
• 2–4 years of clinical social work experience in healthcare setting preferred.

Responsibilities

• Conducts age-appropriate psychosocial assessments.
• Provides counseling and resource education to patients and families.
• Assists with discharge planning and post-acute care coordination.
• Facilitates referrals to internal and external services.
• Manages high‑risk referrals and complex cases.
• Provides crisis intervention for acute distress.
• Collaborates with care teams to support planning and transitions.
• Maintains documentation in the electronic medical record.
