Job summary

Salem Health Hospitals & Clinics is seeking a Master of Social Work Care Manager for Salem Hospital, with responsibilities spanning admission screenings, care coordination, discharge planning and crisis intervention.

The role requires an MSW from an accredited program and Oregon registration within 30 days of hire, plus ongoing engagement with the care management team to support high-risk patients and post-acute transitions.

Qualifications

• Master’s in Social Work from an accredited school required.
• Registration with the State of Oregon within 30 days of hire as either LMSW or CSWA.
• General licensure/registration must be obtained before calling oneself a social worker.

Responsibilities

• Screen patients on admission to identify needs and coordinate care.
• Provide emotional support, crisis intervention and brief counseling.
• Lead discharge planning and ensure post-acute care alignment with resources.
• Participate in complex care rounds and utilization review processes.
• Educate staff on care management principles and maintain documentation.
