Maimonides Medical Center in Brooklyn seeks a Social Worker to provide professional clinical social work and discharge planning services for individuals, families and groups in the outpatient adult psychiatry setting.

You will collaborate with hospital teams, document patient records, coordinate services, and refer to community resources. Requires MSW and NYS Social Work Certification; 1–2 years of medical social work experience preferred.

Formación

• Knowledge of child abuse/neglect reporting rules.
• Experience with two direct practice modalities.
• Excellent oral and written communication skills.
• Excellent interpersonal skills.
• English reading/writing/speaking required; second language preferred.

Responsabilidades

• Identifies, screens and assesses patients and families for social work services.
• Provides ongoing social work treatment for health-related psychosocial problems.
• Leads psychotherapy groups for patients and families.
• Coordinates patient care and discharge planning with interdisciplinary teams.
• Documents social work assessments and treatment in medical records.
