Montefiore Medical Center seeks a Social Worker to join an interdisciplinary team, providing psychosocial assessments and discharge planning for patients and families. You will collaborate on safe, timely treatment plans and maintain confidentiality in line with policies.

The position requires a Master of Social Work and LMSW/LCSW licensure in New York, with 1–3 years of LCSW experience. A dynamic, patient‑centered environment awaits.

Qualifications

• Masters in Social Work required; LMSW or LCSW licensure in NYS.
• 1–3 years LCSW experience preferred.
• Must have MSW from an NYS dept. of education-registered program.
• LMSW or LCSW licensure required for employment.

Responsabilités

• Screen patients for social service intervention.
• Conduct psychosocial assessments with patient, family and interdisciplinary team.
• Develop, implement and validate discharge/treatment plans in collaboration with patient, family and team.
• Operate independently but utilize supervision for problem resolution in discharge planning.
• Maintain confidentiality per hospital and Social Services policies.
• Demonstrate leadership to assist with team building and education.
• Participate in quality improvement, reporting, meetings and related duties.
• Understand ethical issues in end‑of‑life care and advance directives.
• Identify and intervene for victims of abuse.
