Job summary

Montefiore Medical Center in New York City seeks a skilled Social Worker to join an interdisciplinary team and support patient and family psychosocial needs. You will identify barriers to care, assist with discharge planning, and collaborate with clinicians to ensure safe transitions.

The ideal candidate holds an MSW with LMSW/LCSW licensure, 1–3 years of LCSW experience, and a commitment to ethical practice, confidentiality, and ongoing professional development.

Qualifications

• MSW from an accredited program; LMSW or LCSW license required
• 1–3 years of LCSW experience
• Must have NYS LMSW/LCSW licensure and ability to supervise under NYS rules
• If limited permit, execute MOU with licensed Social Worker; obtain LMSW within 6 months
• Fit Test and Annual Assessment required

Responsibilities

• Screen patients for social service needs using standard practices.
• Conduct psychosocial assessments with patient and family.
• Develop and validate discharge/treatment plans with the team.
• Maintain confidentiality per policies.
• Lead, train and support team in psychosocial care and education.
• Participate in quality improvement and staff meetings.
• Address end-of-life issues and surrogate decision making.
