Job summary

Qualifications

• Master’s degree in Social Work from a NYS-registered program or equivalent.
• 1–3 years of relevant work experience.
• Licensed Master Social Worker (LMSW). If holding a limited NY State permit, an MOU with a licensed Social Worker and LMSW status within 6 months is required.
• Ability to operate a personal computer and phone.

Responsibilities

• Screen patients for social work intervention according to practice standards.
• Conduct psychosocial assessments using patient, family, and interdisciplinary data.
• Ensure continuity of care through discharge/treatment plan development.
• Function independently with supervisory guidance as needed.
• Maintain confidentiality per hospital and Social Services policies.
• Develop linkages and networking relationships.
• Participate in patient-focused conferences and internal/external meetings.
• Demonstrate leadership in psychosocial education for staff.
