Job summary

Michigan Medicine is seeking a part-time MSW to join the Adult Inpatient Care Management Team in Ann Arbor. The role involves biopsychosocial assessments, crisis intervention, and facilitating hospital transitions with patient and family-centered care.

Qualified candidates hold an MSW from a CSWE-accredited program and an active LMSW/LLMSW license in Michigan, with experience in hospital social work. The position profiles a flexible, on-site work arrangement with opportunities for collaboration

Qualifications

Master's Degree in Social Work from a CSWE-accredited program is required.
Current license to practice social work in Michigan (LMSW or LLMSW) is required.
Registration, certification or licensure with the appropriate agency as identified on the Primary Source Verification Matrix is required.
Primary Source Verification is required for this classification.
Some experience of professional social work is preferred.

Responsibilities

Conduct biopsychosocial assessments and crisis intervention as part of Care Management Social Work services.
Support hospital transitions and discharges; coordinate with community resources and agencies.
Engage in multidisciplinary team meetings and educational activities.
Assist with end-of-life, grief and loss, and related supports for patients and families.
Participate in quality improvement and patient care planning activities.
