As part of the care management team, you will document assessments, referrals, and plans, work with diverse populations, and help connect patients with community resources and supports.

Competenze

• MSW degree and New Jersey Social Worker license.
• One year of hospital case management or healthcare experience.
• Excellent communication and organizational skills.
• Ability to work with diverse age groups and populations.
