Job summary

Family & Children’s Agency is hiring a MOMS Home Visitor to provide in-home services for at-risk families in the Norwalk area. You will conduct regular home visits, offer case management, and help connect families with essential community resources.

The role emphasizes promoting healthy child development, strengthening parenting skills, and coordinating with schools, health care, and service providers. A CT driver’s license is required.

Qualifications

• Must have awareness of cultural and socioeconomic characteristics of clients served.
• Ability to work as a team.
• Must hold valid Connecticut driver's license.

Responsibilities

• Provide home visiting and case management services to a caseload of 10 to 12 families living in the Greater Norwalk Area.
• Facilitate groups and workshops as assigned; assist with transportation and/or childcare to groups/events as needed.
• Outreach to identify and recruit new clients for the MOMS Program.
• Connect clients with appropriate referrals to community resources, including health and social services.
• Accompany clients to medical and agency appointments as necessary.
• Serve as an advocate for clients with schools, health care, and agency providers.
• Provide accurate and relevant information on child development and child health and wellness.
• Identify client goals and work towards the attainment of client goals.
• Assist client in strengthening parenting skills and problem-solving skills.
• Help clients identify and use their informal support network.
• Participate in orientation and training sessions.
• Attend regularly scheduled supervision and staff meetings.
• Attend community and state-wide meetings and trainings as assigned.
• Complete all assigned paperwork in an accurate and timely manner.
