Job summary

Wellnest is seeking a Home Visitor to support caregivers and newborns up to age 5 at White Memorial Medical Center in Los Angeles. The role follows Healthy Families America, delivering weekly visits initially and longer-term support, with emphasis on attachment, development, and community linkage.

The position requires a Bachelor’s degree in a relevant field, at least two years of case management, and the ability to drive to client homes in varied Los Angeles neighborhoods.

Qualifications

• Bachelor’s Degree in Child Development, Psychology, Social Work or related field with at least 2 years of case management work.
• Home Visiting experience is highly preferred.
• Valid California driver’s license and clean driving record.
• TB test and fingerprint clearance as required.
• Arbitration Agreement as a condition of employment.
• Ability to drive to client homes and community sites daily.

Responsibilities

• Conduct surveys/assessments of parents and caregivers to determine eligibility for the program.
• Provide regular home visits for up to five years, including weekly visits for the first six months.
• Build rapport with families to strengthen parent-child relationships and support child development.
• Refer families to supportive services and assist with goal setting and planning.
• Complete required documentation and maintain charts per organizational guidelines.
