Wellnest Emotional Health & Wellness is seeking a Home Visitor to provide services to caregivers and newborns through age 5 under the Healthy Families America model. You will conduct eligibility assessments and deliver regular home visits to support parent-child attachment and child development.

The role involves building trust with families, linking them to resources, and advocating for child and family well-being. Travel and flexible hours may be required, including weekends.

Competenze

• Bachelor's degree in child development, psychology, social work or related field.
• Two years of case management experience preferred.
• Home visiting experience highly preferred.

Mansioni

• Conduct surveys/assessments of parents and caregivers to determine eligibility for the program.
• Provide regular home visits for program participants for up to five years.
• Provide weekly home visits for at least the first six months after the child’s birth and possibly less frequently after the first six months depending on the family’s progress and challenges.
• Follow guidelines for delivering research-based curriculum.
• Demonstrate the knowledge and skills required to successfully implement the model’s family centered strategies.
• Build rapport with families to establish a trusting relationship with participants; strengthen the parent-child relationship; improve parenting skills; and serve as an advocate for child and family.
• Assist families in referrals and linkage to other supportive agencies.
• Support the family with setting goals and develop a plan for achieving these goals.
• Conduct standardized, periodic screenings to identify potential developmental delays or health challenges facing the child.
• Represent the Clinic while supporting the mission by engaging actively in community events, including events related to recruitment of clients and funder activities, including on weekends.
• Complete required documentation to meet organizational guidelines and standards for client charts.
• Perform other duties as assigned.
