Family & Children’s Center seeks a dedicated professional to support families through the Healthy Families America home-visiting model. The role emphasizes direct service, assessment, and referral development within a community-focused team.

You will interview families, document visits within 48 hours, and participate in ongoing staff development to enhance skills in child development, family dynamics, and preventive care.

Qualifications

• Bachelor’s degree in related field with minimum three years’ relevant experience.
• Working knowledge of child development, behavioral management, and family dynamics.
• Ability to connect families with community resources and teach self-sufficiency skills.

Responsabilités

• Provide direct services to individuals and families referred to Healthy Families program.
• Interview and assess family history and strengths of new referrals.
• Attend and participate in staff meetings.
• Complete 40 hours per year staff development including diversity, CMS training, and safety credits.
• Record visits and activities and maintain documentation within 48 hours of service.
• Develop and maintain referral sources and conduct community outreach.
• Clinical supervision as assigned and appropriate for role.
