Job summary

The Village is seeking an Upward Mobility Mentor/Coach to ensure access to comprehensive, community-based services for clients from referral through treatment in the Greater Hartford, CT region. You will provide linkage and navigation, identify and access Village services, and document progress while maintaining relationships with referral sources and community partners.

Strong communication and cultural competence are essential.

Qualifications

• Excellent oral and written skills and ability to communicate comfortably with children, parents, and professionals.
• Organized and able to work independently with minimal supervision.
• Flexible and able to multi-task.
• Proficient with word processing and spreadsheet applications.
• Bi-lingual English/Spanish strongly preferred.

Responsibilities

• Provides linkage and navigation for clients and families to access community services.
• Helps clients identify and access Village clinical and non-clinical services; conduct referrals and follow-up.
• Maintains and updates service data for reporting purposes.
• Engages clients from referral through transfer or discharge and conducts initial intake.
• Implements Service Plans with clients and tracks targeted goals.
• Conducts client surveys and completes routine program reviews.
• Maintains collaborative relationships with Village programs and community agencies.
• Performs timely documentation and reporting, including entering data into databases.
• Represents the Village in community groups or committees as directed by supervision.
