JOB SUMMARY
The Upward Mobility Mentor/ Coach will ensure that clients haveaccess to comprehensive, community-based services, and that allrequirements from referral sources are met. Focus on engaging clientsfrom the time of the referral through the entire course of theclient’s treatment services.
KEY RESPONSIBILITIES
- Provides linkage and navigation assistance for clients and theirfamilies/significant others so they can access all relevantcommunity services.
- Helps clients and their families identify and access other Villageclinical services/non-clinical services and/or other communityresources. Screening will provide the basis for referrals, andnavigation services will include advocacy and follow-up. OtherVillage services may include specialty treatment for children and adults.
- Maintains and updates service data to ensure accurate reporting,when required.
- Takes steps to ensure client engagement from referral throughtransfer/discharge from service, including initial outreach toclients upon receiving referrals.
- Conducts baseline Intake on all clients upon entering the program.
- Implements Service Plan with client on specific targeted goals toachieve while in program.
- Conducts client surveys/feedback on program.
- Implements service plan reviews with client every (6 months or as necessary).
- Implements discharge service Plan and Client survey of programupon completion of program.
- Provides comprehensive, targeted coaching & case managementservices to clients.
- Completes all required documentation for client while in program(service plans, reviews, progress notes, required data).
- Develops and maintains collaborative relationships with TheVillage Programs and other community agencies, organizations, andreferral sources
- Maintains effective, positive working relationships with referral sources.
- Performs accurate and timely completion of required documentationand reports, including entering client level data into database.
- Meets all standards for timeliness and accuracy in performing dataand reporting functions, including communication with referral sources.
- Comply’s with all agency policies and procedures, HIPAAregulations, Joint Commission standards and funding sources requirements.
- Participate in individual/group/team supervision, clinical qualityimprovement and Quality Assurance activities; participates inUtilization Review.
- Contribute to agency visibility and development of communitylinkages by representing the Village in community groups orcommittees, when directed by Supervisor.
- Perform other duties and responsibilities as assigned by management
Experience and Skills
EDUCATION
High School diploma required, Bachelor’s Degree preferred
EXPERIENCE
- Excellent oral and written skills and ability to communicatecomfortably with children, parents, and both primary care andbehavioral health professionals
- Organized and able to work independently with minimal supervision.
- Flexible and able to multi-task.
- Proficient with word processing and spreadsheet applications.Possess solid general computer skills, including ability to work ina Windows environment. Familiarity with specific applications usedby The Village and its referral sources is a plus.
- Must be culturally competent to serve diverse client population;bi-lingual (English/Spanish) strongly preferred
