Job summary

The Village for Families & Children in Hartford, CT is seeking an Upward Mobility Mentor/Coach to ensure access to comprehensive, community-based services and meet referral requirements throughout the treatment process.

Responsibilities include client engagement, service planning, data entry, and collaboration with community agencies to support diverse client populations, with a strong emphasis on holistic family support services.

Qualifications

• High School diploma required; Bachelor’s degree preferred.
• 2–5 years of relevant experience in behavioral health or related fields.
• Strong written and verbal communication skills; ability to work with diverse populations.
• Bi-lingual (English/Spanish) strongly preferred.

Responsibilities

• Provides linkage and navigation for clients and families to access community services.
• Helps clients access Village clinical and non-clinical services and community resources.
• Maintains and updates service data for accurate reporting.
• Ensures client engagement from referral through discharge and initial outreach.
• Conducts baseline Intake on all clients entering the program.
• Implements Service Plans with clients to achieve targeted goals.
