Job summary

Dermacore Wound Care is seeking a Physician Assistant to join our mobile wound care team on a per diem basis. You will provide hands-on wound care in patients’ homes, exercising clinical independence with an experienced interdisciplinary team.

You will manage assessments, documentation, and treatment plans while traveling within the service area. A valid PA license and national certification are required; mileage reimbursement is provided and W-2, non-exempt employment is offered.

Qualifications

• Active Physician Assistant license in the applicable state.
• Current national certification and prescribing authority, as applicable.
• Strong clinical assessment, critical-thinking, and decision-making skills.
• Excellent communication and documentation skills.
• Ability to independently manage patient care in a mobile environment while collaborating with an interdisciplinary team.
• Willingness to travel throughout the assigned service area.
• Ability to maintain licensing, credentialing, certifications, and continuing education requirements.
• Wound care experience preferred; additional wound care education may be provided.

Responsibilities

• Perform comprehensive patient and wound assessments and develop treatment plans.
• Provide wound care visits per patient’s needs and plan of care.
• Perform wound care procedures within scope and privileges.
• Develop and manage wound-related treatment orders.
• Monitor wound progression and adjust treatment plans as needed.
• Complete accurate clinical documentation with same-day charting expectations.
• Communicate with patients, families, agencies, physicians, and care team.
• Collaborate with Clinical Services, Care Coordination, Medical Records and others.
