Job summary

Dermacore Wound Care is seeking a Physician Assistant to join our mobile wound care team on a per diem basis. You will perform wound assessments, develop treatment plans, and provide in-home wound care with an interdisciplinary team.

You will document thoroughly and communicate with patients, families, and care partners while maintaining HIPAA compliance. This role offers flexibility with per diem scheduling, travel within the service area, and mileage reimbursement.

Qualifications

• Current and unrestricted Physician Assistant license in the applicable state.
• Current national certification and prescribing authority, as applicable.
• Strong clinical assessment, critical-thinking, and decision-making skills.
• Excellent communication and documentation skills.
• Ability to independently manage patient care in a mobile environment with team collaboration.
• Reliable transportation and willingness to travel within service area.
• Maintain licensing, credentialing, certifications, and CE requirements.
• Wound care experience preferred; additional wound care education may be provided.

Responsibilities

• Perform comprehensive patient and wound assessments and develop individualized treatment plans.
• Provide wound care visits based on clinical needs and plan of care.
• Perform wound care procedures within scope and privileges.
• Develop, update, and manage wound-related treatment recommendations and orders.
• Monitor wound progression and adjust treatment plans as needed.
• Complete accurate clinical documentation within required timelines, same-day charting expected.
• Communicate with patients, families, referring agencies, physicians, and care team.
• Collaborate with Clinical Services, Care Coordination, Medical Records, and other teams.
• Recognize changes in patient condition and escalate when necessary.
• Participate in clinical education, training, and team meetings.
• Maintain compliance with HIPAA and applicable regulations.
