Job summary

Dermacore Wound Care is seeking a Physician Assistant for a per diem mobile wound care role. You will perform in-home assessments, develop treatment plans, and provide wound care with an interdisciplinary team. Independent field work with strong documentation is required.

The position emphasizes patient-centered care, flexible scheduling, and collaboration with clinical services. Travel within the service area is expected and mileage reimbursement may apply.

Qualifications

• PA license current and unrestricted in the applicable state.
• National certification and prescribing authority as applicable.
• Strong clinical assessment, critical-thinking, and decision-making skills.
• Excellent communication and documentation skills.
• Ability to independently manage patient care in a mobile environment while collaborating with an interdisciplinary team.
• Reliable transportation and willingness to travel throughout the assigned service area.
• Ability to maintain licensing, credentialing, certifications, and continuing education requirements.
• Wound care experience preferred; additional wound care education may be provided based on experience.

Responsibilities

• Perform comprehensive patient and wound assessments and develop individualized treatment plans.
• Provide wound care visits based on each patient’s clinical needs and established plan of care.
• Perform wound care procedures within your scope of practice and privileges.
• Develop, update, and manage wound-related treatment recommendations and orders.
• Monitor wound progression and adjust treatment plans as needed.
• Complete accurate clinical documentation within required timelines, including same-day charting.
• Communicate with patients, families, and care team about treatment and progress.
• Collaborate with internal teams to promote continuity of care.
• Recognize changes in patient condition and escalate when necessary.
• Participate in clinical education, training, and team meetings.
• Maintain compliance with HIPAA and regulatory standards.
