Job summary

Dermacore Wound Care seeks a Physician Assistant to join our mobile wound care team on a per diem basis. You will perform home-based wound assessments, deliver care per individualized plans, and document thoroughly in real time while collaborating with an interdisciplinary team.

You will work independently in the field with guidance from the care team, travel within the service area, and maintain licensure and certifications.

Qualifications

• License to practice as Physician Assistant in the applicable state.
• National certification and prescribing authority as applicable.
• Strong clinical assessment and decision-making skills.
• Excellent communication and documentation abilities.
• Ability to manage patient care in a mobile, home-based setting.
• Willingness to travel within the service area and maintain licensure.

Responsibilities

• Perform comprehensive patient and wound assessments and develop treatment plans.
• Conduct wound care visits and procedures per plan of care.
• Document visits accurately with same-day charting.
• Coordinate with care team and referring agencies.
• Monitor wound progression and adjust plans as needed.
• Communicate with patients and families about progress and changes.
