Per Diem Physician Assistant – Mobile Wound Care Position
Summary
Dermacore Wound Care is seeking a Physician Assistant (PA) to join our mobile wound care team on a per diem basis. This role provides hands-on, patient-centered wound care in patients’ homes and other community-based settings, combining clinical independence with the support of an experienced interdisciplinary team.
Our providers take ownership of the full patient experience—from assessment and treatment to documentation, communication, coordination, and follow-up. We are looking for a provider who is confident working independently in the field while staying connected and collaborative with the broader care team.
This is a per diem position with flexible scheduling based on provider availability, patient census, geographic coverage, and operational needs.
Patient volume and assignments may vary, and a minimum number of patient visits or hours is not guaranteed. Local travel to patient homes and/or facilities within the assigned service area is required. Travel of up to approximately 55 miles may be required depending on patient location and geographic coverage.
Mileage reimbursement is provided for eligible business travel in accordance with Company policy.
No routine on-call requirement.
The per-visit rate includes the work associated with completing the patient visit, including the clinical encounter, required documentation and charting, communication with agencies and other members of the care team, communication with patients and families/caregivers as appropriate, and routine follow-up associated with the visit.
At Dermacore, Healing at the Core is more than a phrase—it reflects how we approach patient care.
Great wound care goes beyond treating the wound. It means understanding the whole patient, recognizing changes early, communicating with the care team, and following through.
Our goal is to provide safe, effective, patient-centered wound care that supports healing, improves outcomes, and brings quality care directly to the patients who need it.
