Job summary

QuantumCare is seeking motivated Nurse Practitioners or Physician Assistants to join our expanding mobile wound care team. You will provide in-home and in-clinic visits, perform health assessments, and deliver advanced wound care with autonomy and compassionate patient-centered care.

You will manage chronic and acute conditions, document encounters in the EMR, and coordinate with care teams to optimize outcomes while visiting patients at home or in partner facilities.

Qualifications

• Active NP or PA license in Nebraska or Iowa.
• Experience in wound care or home-based care preferred.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide advanced wound care in home or clinic settings.
• Assess, debride, and plan treatment for complex wounds.
• Document patient encounters in the EMR with accuracy.
• Coordinate with care teams to optimize patient outcomes.
• Comply with Medicare/state regulations and organizational protocols.
