Job summary

QuantumCare Wound Care seeks motivated Nurse Practitioners or Physician Assistants to join its expanding mobile wound care team. You will perform in-home and in-clinic visits, assess patients, and manage chronic and acute conditions with advanced wound care techniques.

Role emphasizes relationship-based medicine, autonomy, and a supportive environment. Travel to patient homes and facilities is routine, with a focus on comprehensive, patient-centered care.

Qualifications

• Active and unrestricted NP or PA license in Nebraska or Iowa.
• Experience in wound care, home-based care, or geriatric medicine preferred.
• Reliable transportation and valid driver’s license; day-trip travel required.
• Strong clinical assessment, communication, and documentation skills.
• Self-starter comfortable working independently with mobile technology.

Responsibilities

• Provide advanced wound care treatment for patients with complex or non-healing wounds, including thorough assessment, debridement, and individualized treatment plans.
• Deliver mobile wound care services directly in patients' homes or care facilities, ensuring comfort, accessibility, and continuity of care.
• Diagnose and manage chronic and acute conditions using a comprehensive, whole-patient approach.
• Accurately document all patient encounters, treatments, and progress in the EMR system in accordance with organizational standards.
• Actively participate in care planning and coordinate with internal and external support teams to optimize outcomes.
• Ensure strict compliance with Medicare, state regulations, and organizational protocols governing wound care services.
• Other duties as assigned.
