About Old Mission Advanced Care
Old Mission Advanced Care delivers physician-led, home-based wound care under value-based contracts with health plans. Our clinicians treat patients where healing actually happens — at home — inside small geographic pods, with a nurse navigator coordinating every case, dispatch handling routing, and protocols built on evidence rather than fee-for-service volume. We are launching our Texas and Florida pods now and hiring the founding clinician fleet.
The role
You will be the treating clinician for a defined local pod of homebound wound patients — debridement, advanced wound therapy, vascular assessment, and whole-person care planning — on scheduled days you commit to in advance. A Nurse Navigator owns each patient's continuity between your visits; Dispatch builds your route inside your pod; you document the same day and treat to protocol with real clinical backup.
What you'll do
- Provide in-home wound care within your assigned pod: assessment, sharp debridement, compression therapy, advanced modalities, and in-house vascular diagnostics (ABI/duplex) for stalled wounds
- Manage a standing panel on a set weekly rhythm (your patients see you , not a rotation)
- Document completely the same day — visit time, wound dimensions and debrided area, product and lot for advanced therapies
- Escalate red-flag findings the same day to the National Clinical Director; coordinate with the patient's Nurse Navigator between visits
- Grow with your pod: PRN pods that reach full census convert to a full-time position (see below)
What you bring
- Active, unencumbered NP or PA license in [Texas/Florida], with an active CAQH profile (credentialing must complete before your first visit — we run the process with you)
- You live inside the pod territory — this is a local route, not a travel assignment
- Bilingual Spanish/English required — our patients must be able to talk with their clinician
- Procedural confidence: sharp debridement and multi-layer compression are core skills here (we verify competency in the interview — and we train to our protocol)
- Reliable vehicle and a home base inside the territory
Pay & structure — stated plainly
- $500 per scheduled clinical day (PRN Retainer), $600/day for urgent-coverage days activated by Dispatch
- W-2 employment — taxes, malpractice coverage, and workers' comp handled as an employee, not a contractor
- Conversion path: when your pod reaches full census (~28–30 active patients), the seat converts to **full-time W-2 at a $125,000 base salary plus bonus, benefits and a company auto program (FAVR: $300/month plus mileage).
- Quarterly performance variable pay tied to enterprise results, and Senior Field Clinician designation (with a monthly stipend) for clinicians who take on QA and ride-along duties as pods mature
Why clinicians join us
- Small panels, real continuity, no RVU treadmill — value-based means your outcomes matter more than your visit count
- A navigator and dispatcher between you and the chaos: scheduling, routing, and between-visit follow-up are not your second job
- Founding-fleet seats: the first clinicians in each pod shape how it runs
Who this is not for
clinicians who need a supervising surgeon down the hall, who want a desk and a chart queue, or who are looking for travel-contract variety. This is autonomous, in-home, protocol-driven wound care with the same patients every week — and sharp debridement you stand behind yourself.
