Job summary

Old Mission Advanced Care seeks a Lead Nurse Navigator, RN to own a panel of 75–100 home-based wound patients from first contact to closure, coordinating care with health-plan partners and the plan’s managers.

You will triage acuity, set visit cadence, monitor remote data, track advanced-therapy spend against caps, and train upcoming Navigators. A multistate compact license, bilingual Spanish/English, 4+ years in care management, wound care, and CCM are required or preferred.

Qualifications

• Active RN license with multistate compact privilege (our panels are in TX & FL — both compact states)
• Bilingual Spanish/English required — you are the voice our members hear most; this is clinical, not cosmetic
• 4+ years in care management, home health, wound care, or complex panel management — enough to triage autonomously by phone and data
• Documentation discipline and comfort with remote-monitoring platforms and EHRs
• Wound certification (WCC/CWON) or CCM preferred — and a plus if you've worked value-based or capitated panels before

Responsibilities

• Run structured clinical intakes (about 30 minutes) — history, acuity triage, visit cadence, education, barriers — and set standing appointment windows our Dispatcher routes to
• Run the virtual panel: enroll remote-monitoring patients (cellular scales, wound cameras), execute the monthly monitoring protocol, escalation threshold breaches
• Track cumulative advanced-therapy spend per patient against contractual caps and flag early
• Staff your share of our 24/7 accessibility rotation (stipended)
• Write the Navigator playbook from your own practice in your first 90 days — and train the staff Navigators who follow you

Location: Traverse City, MI HQ

Preferred · Michigan remote with regular travel to HQ · Nationwide remote considered — in that order of preference. Panels are in Texas & Florida (Central/Eastern hours); you must be licensed for the Texas/Florida panels — a compact license, or individual TX/FL RN licenses (Michigan is not a compact state; a HQ-based hire will license in TX/FL individually).

About Old Mission Advanced Care:

Old Mission Advanced Care delivers physician-led, home-based wound care under value-based contracts with health plans. Every patient has a field clinician for visits — and a Nurse Navigator who owns their whole journey between visits. We are launching now, and this is the founding Navigator seat: you will set the standard every Navigator after you is hired into.

The Role:

You own a panel of 75–100 home-based wound patients from first contact to closure. Referrals reach you from our health-plan partners; you make the first clinical call, triage acuity, set each patient's standing visit rhythm, watch their remote-monitoring data between visits, track advanced-therapy spending against plan caps, and serve as our named clinical counterpart to the plan's own care managers. When a wound announces that a patient is decompensating, you're the one who catches it first.

Pay & benefits — stated plainly

• Base salary 95,000–105,000, plus a 400-600/month on-call stipend


• Quarterly variable pay (target 15% of base) tied to panel outcomes and enterprise performance — funded from collected results, certified before payout, and explained in full in your offer documents

Who this is not for:

passive intake processors. You will make binding triage calls that move expensive field assets, tell a health plan a case is stalling, and own a panel's outcomes. If you want a queue that empties, this isn't it.
