Job summary

Powerback Rehabilitation is seeking a Speech Language Pathologist to deliver outpatient rehabilitation in the patient’s home or in facility settings across Vernon, South Windsor, Ellington and Enfield. The role emphasizes personalized care for older adults and children in diverse environments.

The position offers a full-time schedule (35-40 hours/week) with mentorship, CEUs, and visa/relocation support as part of our nationwide program.

Qualifications

• Must hold a valid Speech Language Pathologist license in the state(s) of practice.
• Must have reliable transportation to patient homes for in-home visits.
• Experience in outpatient/home-based rehab settings is preferred.

Responsibilities

• Provide outpatient rehabilitation services in in-home and community settings.
• Evaluate and treat speech, language, and swallowing disorders.
• Develop and adjust treatment plans with patients and caregivers.
• Coordinate care with the interdisciplinary rehabilitation team.
• Document assessments, plans, and progress accurately and timely.
• Supervise CFYs, students, and support personnel as required.
• Order supplies and maintain equipment as needed.
• Promote safety and adapt the environment for patient needs.
