Job summary

Powerback Rehabilitation, LLC is seeking a Speech Language Pathologist for full-time mobile outpatient therapy across Enfield, Granby, Suffield, CT. Roles include in-home evaluation and treatment of older adults in ALFs, ILFs, and community settings.

Responsibilities include delivering speech-language pathology services, coordinating with the rehab team, and maintaining documentation. Mentorship for new grads and CEUs are offered.

Qualifications

• License to practice as Speech-Language Pathologist in the state(s) of practice.
• Valid driver's license or reliable transportation to and from patients' residence.

Responsibilities

• Provide outpatient rehabilitation services in home or facility settings for older adults.
• Evaluate and treat speech-language disorders and swallowing.
• Coordinate care with interdisciplinary rehabilitation team and physician.
• Maintain timely clinical and billing documentation and participate in meetings.
• Supervise CFYs and students as applicable and in accordance with licensure requirements.
