Job summary

Your Health is seeking a Registered Nurse to serve patients across the Rock Hill area as a Nurse Case Manager. You will travel to patients’ homes and facilities, coordinate with providers, and initiate telemedicine visits to ensure quality, compliant care.

As part of a growing primary care team, you will enroll patients in Care Management, develop Plans of Care, review records, and collaborate with interdisciplinary teams to optimize outcomes.

Qualifications

• Must be a Registered nurse with license in good standing.
• Minimum of three (3) years clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with interdisciplinary care teams, patients, and caregivers.
• Strong written and verbal communication skills.
• Basic computer knowledge.
• Ability to manage and demonstrate effective leadership skills.
• Flexibility to work varied hours and travel between offices/facilities; licensed driver with insured vehicle.

Responsibilities

• Facilitate receiving all medical records from the patient’s primary provider and specialists.
• Review medical records.
• Complete consents with patients.
• Enroll patients in Care Management if eligible.
• Initiate a Care Management Plan of Care if eligible.
• Capture diagnoses with high specificity and close gaps.
• Complete AWVs and cognitive impairment screenings.
• Complete Social Determinants of Health (SDoH) assessments or screenings.
• Evaluate for home health, hospice, palliative, or consults with Your Health Specialty Division.
• Coordinate with health care team, providers, therapists, home health or hospice representatives.
• Facilitate telehealth visits when necessary.
• Document and log Care Management activities; keep plans up to date.
• Coordinate with patient’s care team across settings.
