Job summary

Commonwealth Care Alliance seeks a Mobile RN Care Manager for Massachusetts to deliver in-home care management for dually-eligible enrollees with complex needs. You will coordinate medical, behavioral and social services, lead the ICT, and ensure seamless care transitions.

The role emphasizes community-based assessments, care planning, and collaboration with primary care and specialists to improve outcomes and quality of life for Members.

Qualifications

• Associates of Science in Nursing from an accredited program.
• Current unrestricted MA RN license required.
• Experience in care coordination or case management preferred.
• Medicaid/Medicare managed care experience preferred.
• Clinical field/community-based training is a plus.

Responsibilities

• Engage enrollees in their homes and community settings to develop care plans.
• Liaise between healthcare providers, resources, and enrollees to ensure care transitions.
• Perform assessments including comprehensive assessments and functional assessments.
• Lead the interdisciplinary care team to create holistic care plans.
• Coordinate with community organizations to avoid service duplication.
• Monitor enrollee utilization of long-term services and supports.
• Educate members about benefits under Medicare and Medicaid.
• Follow up after hospitalizations to ensure continuity of care.
