Job summary

Commonwealth Care Alliance is seeking a Mobile RN Care Manager for the Western, MA region. This community-based role focuses on monitoring, follow-up, and clinical care management for dually-eligible enrollees with complex needs, coordinating with healthcare providers and social services.

The position requires MA RN licensure, care coordination experience, and travel within the territory. A sign-on bonus is offered and the role is full-time with a hybrid work model.

Qualifications

• Current unrestricted RN license in Massachusetts.
• Experience in care coordination or case management preferred.
• Knowledge of Medicaid/Medicare managed care a plus.

Responsibilities

• Engage enrollees in home and community settings to build care relationships.
• Coordinate with providers and resources to ensure seamless care transitions.
• Conduct comprehensive assessments and support care plans.
• Lead interdisciplinary care team to address medical and social needs.
• Facilitate access to housing, transportation, and social services.
• Educate members on benefits, self-management, and preventive care.
• Travel to member and provider sites as required.
