Job summary

PARS Therapy in Rosharon, TX is seeking a compassionate Physical Therapist (PT) to join our home health team. The role focuses on helping patients regain movement, reduce pain, and improve quality of life in the comfort of their own homes.

You will evaluate patients, develop individualized plans, educate caregivers, and collaborate with an interdisciplinary team. A TX PT license and experience in home health are preferred; we offer benefits and flexible scheduling.

Qualifications

• Active Texas Physical Therapist license (TX)
• Professional liability insurance coverage for TX license
• Graduate degree or higher in Physical Therapy
• Experience in home health or rehabilitative settings preferred
• Strong ability to adapt care approaches to individual patients
• Up-to-date knowledge of evidence-based techniques and best practices
• Excellent communication and professional demeanor with patients and caregivers

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting
• Develop and implement individualized treatment plans to restore function, mobility, and reduce pain
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies
• Monitor patient progress and adjust plans to achieve optimal outcomes
• Accurately document evaluations, treatments, and progress notes using digital platforms
• Collaborate with physicians, nurses, and occupational therapists
• Conduct home safety assessments and make recommendations to prevent falls or re-injury
• Deliver compassionate care while empowering patients to regain independence
