Job summary

Pars-Therapy-1 is seeking a compassionate Physical Therapist (PT) to join our home health team in Needville, TX. The role focuses on evaluating and treating patients in their homes to improve mobility and reduce pain.

The ideal candidate holds an active TX PT license, a graduate degree in Physical Therapy, and has home health or rehabilitative experience. We offer comprehensive benefits, 401(k), PTO, and a supportive team with flexible scheduling.

Qualifications

• Active Physical Therapist license in TX
• Graduate degree in Physical Therapy from an accredited program
• Experience in home health or rehabilitative settings preferred
• Strong communication and patient-education skills
• Evidence-based treatment knowledge and clinical reasoning

Responsibilities

• Evaluate patients in a home health setting and determine functional limitations
• Develop and implement individualized treatment plans to restore function and mobility
• Provide interventions including exercise, gait training, manual therapy, and balance retraining
• Educate patients and caregivers on home exercise programs and safety
• Document evaluations, treatment sessions, and progress notes accurately
• Collaborate with physicians, nurses and other therapists
• Conduct home safety assessments and prevent falls or re-injury
• Deliver compassionate care and empower patient independence
