Job summary

PARS Therapy in Houston, TX seeks a compassionate Physical Therapist (PT) to join our home health team. You will evaluate patients at home, develop personalized treatment plans, and implement interventions to improve mobility and reduce pain.

Strong communication with patients and an interdisciplinary team is essential. The role requires a TX PT license, graduate degree in PT, and preference for home health experience.

Qualifications

• Active license to practice Physical Therapy in TX.
• Graduate degree in Physical Therapy from an accredited program.
• Home health or rehabilitative care experience preferred.

Responsibilities

• Evaluate patients in home health setting and assess functional abilities.
• Develop and implement individualized PT treatment plans to restore function and mobility.
• Provide interventions including exercise, gait training, manual therapy and balance retraining.
• Educate patients and caregivers on home exercise programs and safety.
• Monitor progress and adjust plans to achieve optimal outcomes.
• Document evaluations, treatments and progress notes in digital systems.
• Collaborate with physicians, nurses and other therapists.
• Conduct home safety assessments to prevent falls or injuries.
