Job summary

PARS Therapy is seeking a compassionate Physical Therapist (PT) to join our home health team in Houston, TX. You will evaluate patients, create individualized plans, and provide evidence-based therapies in patients' homes.

This on-site role requires a TX license, liability insurance, and a DPT. Prior home health experience and strong communication are highly valued. We offer comprehensive benefits and a team-focused environment.

Qualifications

• Active Texas PT license is required.
• Professional liability insurance coverage required.
• Graduate degree in Physical Therapy (DPT) from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Strong ability to adapt care approaches to diverse patients.
• Up-to-date knowledge of evidence-based treatment techniques.

Responsibilities

• Evaluate patients in a home health setting to determine functional limitations.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
• Monitor patient progress and adjust plans to achieve optimal outcomes.
• Document evaluations, treatment sessions, and progress notes in digital platforms.
• Collaborate with physicians, nurses, and other therapists as part of an interdisciplinary team.
• Conduct home safety assessments and recommend fall-prevention strategies.
• Deliver compassionate, patient-centered care to empower independence.
