Powerback Rehabilitation To You in Greenfield, MA is seeking a Licensed Physical Therapist Assistant for PRN outpatient/home-based rehab to older adults. The PTA will work under supervision of a Licensed PT, delivering home-based therapy and outpatient rehab in various settings, with flexible scheduling.

Responsibilities include implementing individualized treatment plans, documenting, coordinating with the rehab team, and ensuring high-quality patient care.

Qualifications

• Must have a valid license in the state of practice (PTA).
• Must have a valid driver's license or reliable transportation to patients’ residences.

Responsibilities

• Attends and contributes to patient care and staff meetings as a representative of physical therapy.
• Adheres to department and patient treatment schedules; completes timely clinical and billing documentation.
• Collaborates with rehab team to implement patient-centered programs.
• Assists the Physical Therapist during tests, evaluations, and complex procedures.
• Instructs patients and caregivers in treatment techniques under supervision.
• Maintains proper documentation for all patients treated.
• Cleans and maintains equipment; reports need for repairs.
• Puts patient service first with compassionate care.
• Implements individualized treatment plans and provides therapies.
• Performs other related duties as required.
