Job summary
VNS Health is seeking a Psychiatric Registered Nurse to perform mental health assessments, develop treatment plans, and deliver home-based care to patients with psychiatric symptoms and cognitive challenges. Qualifications
- License and current registration to practice as a Registered Nurse in NYS required.
- Able to travel to patients' homes and facilities.
- Experience in psychiatric, cognitive or behavioral health settings preferred.
Responsibilities
- Conduct psychosocial assessments and develop treatment recommendations.
- Provide nursing care and crisis intervention per policy and plan.
- Support and educate staff and caregivers to improve patient care.
- Participate in interdisciplinary team meetings and care planning.
