Job summary

S:US is seeking a Psychiatric Nurse Practitioner to join a dynamic ACT team and Medical Director. You will implement interventions, assess outcomes, and advance person-centered psychiatric care using evidence-based practices.

You will write prescriptions, manage medications, document histories, and coordinate with community psychiatrists. The role requires NY NP license, DEA, and Medicaid provider status; in-person work with the opportunity to contribute to housing and recovery services.

Qualifications

• Licensed as a Registered Nurse in New York State and credentialed as an advanced Nurse Practitioner (psychiatry).
• Must be registered as a NY State Medicaid provider and possess a DEA number and an NPI number.

Responsibilities

• Coordinate ongoing psychiatric treatment with the ACT team and Program Director.
• Write prescriptions for psychotropic medications per state regulations.
• Provide medication management and education to consumers.
• Document medical and psychological histories, assessments, diagnoses, and plans.
• Prepare Psychiatric Assessments bi-annually and for housing/other applications.
• Provide training to ACT team on medications and interactions.
• Coordinate care with community psychiatrists and emergency services as needed.
• Refer residents for emergency care and coordinate discharge planning.
• Maintain timely and accurate consumer chart documentation.
• Attend daily and weekly meetings, supervision, and trainings.
• Comply with SUS policies and procedures and corporate compliance rules.
• Perform all other duties as assigned.
