Job summary

Horizon House, Inc. seeks a Board Certified Licensed Registered Nurse (PA) to coordinate behavioral and physical health care for service participants.

The role involves monitoring medications and providing psychiatric nursing services under physician orders, with about 75% of duties in the community. You will join an Assertive Community Treatment team to help participants reach their maximum functioning in the community and require strong communication, organizational skills, and a valid PA

Qualifications

• Board Certified Licensed RN in PA.
• Experience servicing community mental health facilities and psychiatric needs.
• Knowledge of psychiatric, clinical and psychosocial rehabilitation practices.
• Understanding of Assertive Community Treatment (ACT) model.
• Excellent verbal and written communication with diverse stakeholders.
• Strong organization and analytical skills.
• Travel availability with valid driver's license.

Responsibilities

• Coordinate behavioral and physical health care needs of participants.
• Monitor participant medications and provide psychiatric nursing services per physician orders.
• Participate in Assertive Community Treatment team to support community functioning.
• Approx. 75% of work in community settings.
