Job summary

Ennoble Care seeks an experienced Full-time Provider (Nurse Practitioner or Physician Assistant) to deliver primary and palliative care to patients in the Destin, FL region. You will assess and treat chronic diseases under physician supervision, providing home visits and coordinating with a dedicated clinical team.

The ideal candidate is Florida-licensed, board-certified as NP or PA, with 1–2 years of primary care experience (geriatric care preferred).

Qualifications

• Board-certified NP or PA licensed in Florida.
• Strong clinical skills to assess, diagnose and treat.
• Excellent communication with patients and staff.
• Ability to work independently and problem-solve.
• 1–2 years in primary care with geriatric patients preferred.

Responsibilities

• Provide high-quality primary and palliative care at patients’ homes in/around Destin, FL.
• Perform physical exams and develop care plans.
• Communicate with clinical operations across platforms.
• Attend quarterly medical staff meetings.
• Develop impressions, diagnoses and treatment plans.
• Consult with patients, families, and caregivers; coordinate orders and testing.
