Job summary

Central City Concern in Portland, OR is seeking physicians, nurse practitioners, or physician assistants to join the Mobile Health program and deliver holistic, patient-centered care to people experiencing homelessness.

Role emphasizes whole-person medicine, collaboration with pharmacists and specialists, and reducing barriers to treatment, with on-site and off-site support and a focus on trauma-informed practice.

Qualifications

• Experience in primary care medicine preferred.
• Must have full prescriptive powers in Oregon with a DEA number.
• Current BLS certification required.
• Wound care experience highly preferred.
• Spanish language fluency preferred.
• Valid driver’s license preferred; DMV background check if driving is required.

Responsibilities

• Deliver comprehensive, patient-centered services as part of a primary care team.
• Triage, diagnose, and manage acute and chronic illnesses.
• Coordinate with CCC programs for Hepatitis C, withdrawal management, and mental health.
• Support quality improvement and training initiatives.
• Provide after-hours coverage on rotation.
