Job summary

The Night Ministry seeks a Nurse Practitioner to deliver high‑quality medical care through a harm‑reduction framework to homeless and underserved populations. Working across Street Medicine, Health Outreach Bus, CTA outreach, shelters and youth programs, you will connect clients to ongoing primary and specialty care and community services.

Qualifications

• MSN or DNP from an accredited program
• Licensed Advanced Practice Nurse in Illinois (Family preferred)
• DEA license or eligibility to obtain
• Minimum 2+ years of clinical experience; primary care or community health preferred
• Experience with homelessness, SUD, serious mental illness, or trauma preferred
• Ability to work evenings and weekends
• Valid driver’s license and good driving record

Responsibilities

• Provide low-barrier, harm-reduction medical care across TNM outreach settings
• Conduct focused assessments and treat acute and chronic conditions in mobile/outdoor environments
• Initiate and manage pharmacologic treatment, including MAT
• Provide wound care, vaccinations, preventive services, and health education
• Facilitate referrals and warm handoffs to primary/specialty care and social services
• Maintain accurate documentation in EHR and reporting systems
• Collaborate with interdisciplinary outreach staff to coordinate care
• Support medical supply management and medication inventory
• Provide clinical guidance to learners and volunteers
• Participate in quality improvement, training, and meetings
• Travel to outreach locations and work in outdoor environments
• Perform additional duties as assigned
