Job summary

Central City Concern is seeking a Primary Care Provider (MD, DO, NP or PA) for flexible scheduling to deliver comprehensive primary care in a dynamic, multi-site setting. The role emphasizes harm reduction, whole-person care, and collaboration with pharmacists and nursing teams.

You’ll work in a team-based environment across CCC programs, with opportunities to teach and mentor colleagues while shaping clinical workflows.

Qualifications

• Board eligible or certified in a primary care specialty relevant to our services.
• Licensed to practice as a Physician in Oregon.
• Graduate of an accredited Nurse Practitioner Program with appropriate scope and unrestricted licensure in Oregon or eligibility for such licensure.
• Appropriate board certification for NP/PA as applicable.
• Graduate of an accredited Physician Associate Program with unrestricted licensure in Oregon or eligibility for such licensure.

Responsibilities

• Deliver comprehensive, patient-centered primary care as part of a multidisciplinary team.
• Take health histories, perform exams, review vitals and labs, prescribe medications, and manage acute and chronic conditions.
• Triage, diagnose, and manage minor illnesses with a focus on engagement and continuity of care.
• Support patients in resiliency through shared decision making, motivational interviewing, and relationship-based care.
• Coordinate with CCC programs across Hepatitis C treatment, withdrawal management, and mental health services.
