Job summary

Your Health Organization is seeking a motivated Physical Therapy Assistant to join our home-based care team in South Carolina. You will work closely with licensed PTs to provide rehabilitative services to homebound patients, conducting exercises, educating patients, and documenting progress to support recovery and discharge planning.

The role covers visits to homes and facilities in Conway and surrounding areas, with a Monday–Friday schedule and a focus on patient safety and high-quality care.

Qualifications

• Must have PTA licensure or be eligible for licensure in the state of practice.
• Associate degree from an accredited PTA program.
• CPR certification and strong interpersonal skills.

Responsibilities

• Assist licensed physical therapists in delivering rehabilitative services to patients.
• Implement treatment plans and conduct therapeutic exercises with patients.
• Educate patients on proper rehabilitation techniques and document progress.
• Develop individualized treatment plans and monitor progress during therapy sessions.
• Maintain a clean, organized therapy environment and sanitize equipment.
• Coordinate care with other healthcare professionals and assist in discharge planning.
