Responsibilities

• Obtain complete health and psychosocial history and record findings.
• Perform complete physical assessment and provide health education and counseling.
• Arrange referrals to other health team members and community resources.
• Use reporting mechanisms as required by law and protect patient privacy and consent for treatments.
• Market and provide community education.
