Best Home Health Providers, Inc. is seeking a per diem Physical Therapist to deliver skilled home health services across the Bay Area and beyond, helping patients regain mobility and independence in their homes.

This role requires a California PT license, at least 2 years of PT experience, and a valid driver’s license with reliable insured vehicle. Per visit compensation ranges from $160 to $180, with potential for full-time conversion based on performance.

Qualifications

• Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.

Responsibilities

• Evaluate patients' functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician's plan of care and California's Physical Therapy Practice Act.
• Educate patients and their families on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.
