UnitedHealth Group is seeking a Physical Therapist for Home Health to perform patient assessments, develop care plans, and deliver interventions aimed at improving well-being in a home setting. You will monitor progress, document outcomes, and collaborate with a healthcare team to coordinate care.

The role requires current PT licensure in the state of practice, a valid driver’s license, and access to reliable transportation. CPR certification is preferred within 90 days of hire.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsabilités

• Evaluates the patient's functional status and PT needs.
• Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice per state laws and plan of care.
• Observes and reports patient response to treatment to the supervising nurse/physician.
• Treats patients to relieve pain and restore function to maximum functioning.
• Instructs patients and family/caregivers regarding plan of care and progress.
• Completes progress notes and PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and frequency of care.
• Function in diverse home settings regardless of background.
